Reporting A Claim:
The main goal of the telephone first notice is to promptly and accurately report the claim. This alerts all parties involved of the injury so that you may receive the appropriate care and attention.

Stop; turn on your emergency flashers and shut off your vehicle; do not move your vehicle until the police arrive |

Assist the injured but do not move anyone; wait for medical assistnace if needed |

Call the police and your company; request medical assistance if needed |

Use a nearby phone or CG, or ask a passerby to make calls for you; use the "Emergency Assistance Request" card |

| Step 3: Accident Documentation |

Use "Courtesy Information Cards" to collect information from witnesses; be polite and courteous; don't admit guilt or apologize |

If you strike an unattended vehicle and cannot locate the owner, leave a note with your name, address, and company name securely on the vehicle; get the vehicle's ID number and license number. |

Give your name, address, company name and address, vehicle license number, operator's license and insurance information to the police and other party involved |

Fill in the "Preliminary Accident Report" at the scene and take pictures of the general scene, the vehicles (include a view with the license tag), and your cargo. Refer to the instructions and photo hints on the accident kit package |

Don't sign anything or make any statements except to the police, your company, or insurance company |

Secure your vehicle from theft and further damage; remain at the scene until all requirements are met |
